• Delicate and finely vacuolated cytoplasm
• Intranuclear cytoplasmic inclusions
• Intracytoplasmic lumens with mucin
• Psammoma bodies (papillary subtype)
• Enlarged and irregular
• Prominent nucleoli, often cherry red macronucleoli
• Open chromatin
• Mucinous, non-mucinous, papillary, clear cell, colloid, hepatoid
• Positive for cytokeratin, CK7, TTF-1 (may be negative in mucinous), NapsinA; negative for CK20 (may be positive in mucinous), CDX2
• May have mutations in EGFR, KRAS, BRAF; rearrangements in ALK, ROS1
