Features

Architecture

• Solid monotonous sheets, cystic, papillary growth pattern
• Resembles normal acinar cells
• Bunch of grapes appearance
• Cellularity with acinic cells only (no ducts or stroma)

Cellular features

• Acinic cells with granular or vacuolated cytoplasm
• Clear cells with clear cytoplasm (like RCC)
• May have intercalated ductal cells and glandular cells

Nuclear features

• Nuclei round and uniform
• May have binucleation
• Naked nuclei may be present
• Fine to coarse chromatin
• May have prominent nucleoli
• May have intranuclear cytoplasmic inclusions

Background

• Mitosis and necrosis typically rare
• May have chronic inflammation
• May have psammoma bodies
• Typically stroma or mucin no present

Ancillary studies

• Express cytokeratin, zymogen granules PAS positive (PTAH negative D/D oncocytes)
