Features
• Body-cavity based large B-cell lymphoma associated with human herpesvirus (HHV-8) without any detectable tumor mass. Often seen in setting of immunodeficiency e.g. HIV infection or rarely post-transplant. Often co-infection with EBV.
• Dispersed large markedly atypical lymphoid cells
• Plasmablastic or immunoblastic to anaplastic
• Moderate to abundant basophilic cytoplasm
• Cytoplasmic vacuoles may be present
• Large round irregular nuclei
• Anaplastic multilobated nuclei may be present
• Prominent nucleoli
• May have perinucleolar hof
• Mitosis present
• Apoptotic bodies may be seen
• IHC: Usually express CD45, but negative for CD19, CD20 and CD79a, surface immunoglobulin. Always positive for HHV8, usually positive for EBV by EBER ISH but negative for EBV LMP1. Morphologically similar to diffuse large B cell lymphoma and IHC help differentiate.
