Specimen

• Usually seen as bile duct brushings/ washings
• Cytologically similar to ductal epithelial cells of pancreatic duct

Architecture

• Cohesive flat orderly honeycomb sheets
• Palisading of cells at the edge
• Rare single intact cells

Cellular features

• Uniform cuboidal cells
• Moderate to abundant delicate cytoplasm
• Goblet cells may be present

Nuclear features

• Round to oval nuclei
• Smooth nuclear membranes
• Basally oriented nuclei
• Fine chromatin
• May have small nucleoli

Background

• Granular green-gold bile pigment present
• Mitosis/necrosis usually not present

Reactive Changes

• Usually in response to inflammation, instrumentation, stents, stones
• Crowding and overlapping of cells
• Nuclear enlargement
• Low N:C ratio
• Smooth nuclear membranes
• Prominent nucleoli
• Mitoses frequently seen
