• Clusters, sheets and single cells

Cellular features

• Large cells
• Abundant vacuolated cytoplasm
• May have areas with granular cytoplasm
• May have metachromatic basement membrane• like material

Nuclear features

• Small round to large pleomorphic nuclei
• Eccentrically located nuclei
• Naked stripped nuclei
• Smooth to irregular nuclear membranes
• Fine to coarse chromatin
• Inconspicuous to prominent nucleoli
• May have intranuclear cytoplasmic inclusions

Background

• Hemorrhage
• Necrosis
• May be cystic
• May have calcification

Fuhrman nuclear grade

• Grade 1: Round uniform nuclei (10um), inconspicuous nucleoli
• Grade 2: Slightly enlarged irregular nuclei (15um), conspicuous nucleoli (visible at 400x);
• Grade 3: Moderately enlarged irregular nuclei (20um), prominent nucleoli (visible at 100x)
• Grade 4: Markedly enlarged and bizarre nuclei (>20um), macronucleoli (>RBCs)

Ancillary studies

• Express cytokeratin, EMA, vimentin, CD10, RCC; negative for CK7, CD117
• Loss chromosome 3p, mutation VHL gene
