Cellular features

• Predominantly intermediate-sized cells
• Moderate amount of dense basophilic cytoplasm
• Cytoplasmic vacuoles seen on Diff-Quik-stained smear

Nuclear features

• Nuclei round to oval
• Chromatin finely clumped and dispersed
• Multiple basophilic nucleoli

Background

• Necrosis
• Numerous mitotic figures
• Tingible body macrophages ingesting apoptotic debris

Immunoperoxidase Stains

• Typically positive for Pan-B-cell markers, CD10, Bcl-6; Mib-1 positive in nearly 100% of cells
• Negative for CD5, Bcl-2, CD23, TdT

Flow Cytometry

• Typically express CD45 (bright), CD19, CD20, CD10, CD38, FMC7; kappa or lambda light-chain restriction
• Negative for CD5

Molecular and cytogenetic findings

• MYC translocations
• Partners with Ig gene loci: IgH, Ig-kappa, Ig-lambda
• t(8;14)(q24;q32)- 80%
• t(8;22)(q24;q11)- 15%
• t(2;8)(q11;q24)- 5%
