• Monotonous population of oncocytic cells
• Single cells, loose clusters, round nests
• Large round to polygonal cells
• Granular cytoplasm
• Well-defined borders
• Small round nuclei
• Eccentrically located nuclei
• Fine chromatin
• Binucleation
• Conspicuous to prominent nucleoi
• Mitoses absent
• Necrosis absent
• Often best to diagnose as oncocytic neoplasm on FNA and small biopsy; Need to differentiate from chromophobe CA and RCC with granular features
• Express cytokeratin, EMA, CD117; typically negative for vimentin, CK7, Hales colloidal iron (D/D chromophobe ca).
• May have loss of chromosomes 1 or Y, or translocation 11q13
