• High nucleus to cytoplasmic ratio is essential

• Hyperchromasia (ink-blot nuclei)
• Squamous differentiation may be seen in high grade urothelial carcinoma and if features are focal, diagnosis of keratinizing squamous cell carcinoma should not be made.
